Active Screening Questionnaire for COVID-19 

As of March 21st, we are no longer required to ask families and participants to physically fill out a Daily Active Screening Form prior to participation in a recreational or child care program. 

Daily health screening remains an important tool in preventing the spread of Covid-19. Please consult the Ontario Self Assessment Tool, or the Covid-19 school screening

On-Site Confirmation of Daily Active Screening:  As of March 21st, we are no longer required to ask families to physically fill out a Daily Active Screening Form.  Individuals should continue to self-screen every day before attending childcare by using the COVID-19 school screening (ontario.ca). We will continue to do a daily observation of all children as they arrive and call home if a situation arises where your child is not feeling well.
