Active Screening Questionnaire for COVID-19 

Active Screening is a health review.  Completing Active Screening BEFORE each participation in a recreational or child care program program is a mandate of the Ministry of Health, Ministry of Education, and Ottawa Public Health. Submitting this Questionnaire electronically to the Manor Park Community Council (MPCC) makes it easy for you to comply.

It helps everyone remain safe and healthy!

Participants, adults or children, who have not completed the Active Screening Questionnaire prior to arrival at their program -- 'Before- and After-School in Manor Park', sports, fitness, arts -- will be excluded from participantion.  Participants who cannot submit a clear Active Screening Questionnaire should remain at home, complete a Covid-19 Self-Assessment and follow instructions for testing. 

Information collected will be used by the Manor Park Community Council for the administration and management of child care, arts, sports, and fitness programs. Information will only be shared as legally required.

Summer Day Camps in Manor Park: Daily Active Screening Questionnaire

Required for all children participating in Summer Day Camps in Manor Park, and must be completed each day prior to arrival

Summer Day Camps in Manor Park: Pre-Program Declaration of Health

Required for all children participating in Summer Day Camps in Manor Park, and must be completed prior to each week of attendance.

Summer Day Camps in Manor Park: Staff Daily Active Screening Questionnaire

Required before arriving for work.

Adult Participants:  Active Screening Questionnaire

Required for all adults participating in in-person programs, and should be completed on the day of, each class.

Children: Active Screening Questionnaire

Recreational participants should use the questionnaire above, and submit it before attending in-person programs.

Before- and After-School in Manor Park.

Questionnaire must be received before 7am for before-school care and before 1pm for after-school care.
