COVID-19 Active Screening Questionaire 

Through July and August ALL participants enrolled in MPCC recreational programming are required to complete an online COVID-19 active screening questionaire prior to their arrival. This includes Summer Day Camps, Children's Sports and Adult Fitness. 

Completing this form for yourself or your child will help us to ensure the safety of all participants and staff. 

Information collected will only be used by the Manor Park Community Council (MPCC) for the administration and management of 'Physically Active and Physically Distanced Programs' and will only be shared if legally required.

Active Screening Questionaires must be filled out the day of the program, prior to arrival. Participants (adults or children) who have not completed the Active Screening Questionaires will not be able to participate.
